Revised Whole Genome Sequencing (WGS) test order forms are now available nationally as of 6th May 2026.
The changes align with the recent National Genomic Test Directory (NGTD) updates from 8th April (NHS England » National genomic test directory) and have been informed by extensive national consultation with requesting clinicians.

Please download the updated forms and use them for all future WGS test ordering. These forms are only to be used for WGS testing, for any other testing please continue to utilise your local laboratory referral forms.
Notable changes to WGS Test Order Form
- New field to provide reason for testing such as patient management, reproductive decision making, or unaffected relatives seeking predictive testing. This field is a requirement for testing and will result in delays to patient testing if not completed upfront.
- Penetrance selection field has been removed. All patients will be processed with incomplete penetrance selected from 6th May, including referrals received prior to this date but not yet sequenced.
- The responsible clinician field has moved to the end of the form on page 2. Clinicians may choose to fill this field and save the form locally to avoid having to repeatedly fill for each patient, if appropriate. It is important that all parts of the form are completed prior to sending to the laboratory.
- Requirement to provide information on biological sex to aid quality control during testing has been added instead of gender.
- Modification of the human phenotype ontology (HPO) table to include ‘Unknown’ as an option.
- Updated list of panels in line with the latest NGTD updates.
